How large are the files allowed to be?  I thought the file format for the .tsv conversion had to be mp4.  Every time i try to convert a file using the tsv converter it locks up.  Ive left the process alone for a while just in case... to no avail.  Any ideas?

Check these few things:

• The title cannot be longer than 50 characters
• The title cannot include characters/symbols outside of A-Z, 0-9
• Some videos could be corrupted from the original mp4/source file - make sure the source video is playable on your device
• Some source files can be protected by a DRM - make sure you have the ability/license/permission to alter or re-distribute the video
